The Registry last updated the Form A – List of Office Bearers in April 2024.  Co-op should first read the “Impt Notes” before providing the details in “Section 1 – List of Officers” and “Section 3 - Declaration”.  Credit co-ops should also complete ‘Section 2 – Credit co-ops only”.
